Hi! I set up a form for clients to fill out that is working BEAUTIFULLY. The completed signed form is auto-emailed to me (outlook) by Jotform. The emailed form is named with the [name of document] [submission id]. There is no client identifier at all. However when I download the PDF through the site, it has the name in the title [Client Name] [Date]. How can I get the emailed auto version to come thorugh with a client name on it?

Hi Misty,
Thanks for reaching out to us for help. I'm an AI assistant. I'll do my best to help you until someone from the support team is available.
To have the emailed auto version of your completed form include the client name in the file name, you need to customize the PDF attachment settings in your form's email notifications. Let me walk you through it:
- Open your form in Form Builder and go to the Settings tab.
- Click on Emails on the left menu, then select the Notification Email that sends the completed form.
- Edit the Notification Email by clicking the pencil icon.
- Go to the Advanced tab and toggle the PDF Attachment option to On. Choose the PDF file you want to attach, which you can customize in the PDF Editor to include the client name in the title.
- Save your changes.
This way, the emailed PDF attachment will have the client name as part of the file name, similar to how it appears when you download it from the site.
Give it a try and let us know how it goes.

Hi Misty,
I cloned your form and tried to replicate the issue, but everything worked the way it should. Check out my results in the screenshot below:
Can you try removing the form fields in the file name and adding it again? Let me show you how to do that:
- In Form Builder, in the blue navigation bar at the top of the page, click on Settings.
- Then, in the menu to the left side of the page, click on Jotform Sign.
- In the File Name section, click on the Pencil icon on the right side of the File Name field to edit it.
- Enter a name or click on the Form Fields Dropdown menu to select a Form Field Name, and that’s it.
Give it a try and let us know how it goes.
