Need assistance with this message: "Your forms will be disabled
We’ve detected that you’re collecting personally identifiable information (PII) via your forms. If you do not verify your account by September 14, 2025, your forms will be disabled. If you have any questions, please contact us."

The form has been temporarily disabled due to the collection of sensitive data, such as Personal ID Information. Collecting PII without verification is not allowed. Our top priority is to ensure the security and privacy of all our users. Also, the account owner should receive an email about filling out a questionnaire to verify their organization. If you received an email about your form(s) collecting PII, please note that account verification is required to comply with our Terms of Use. To verify your account and become a KYC-approved user, follow the instructions provided in the email.

Hi Nicole,
Thanks for reaching out to Jotform Support. We take our user's security and privacy very seriously. KYC is a process we developed to help legitimate organizations collect PII while following our terms. Collecting PII without verification is not allowed. KYC is only required once.
If forms include fields that collecting sensitive information, we'll need to verify the organization and the account owner. You can check out our Terms, Privacy and Security pages and the email subjected as "Action Required: Your forms will be disabled if you won't provide us with some key information" for more information about it. If you believe that there's a misunderstanding, let us know by replying back to this thread.
Once you fill out the KYC form, we'll review it and let you know about your forms' status.

Manilene, I have reviewed all 3 pages and searched for KYC forms and this text was not found. Please tell me exactly how I can clear this issue / find and submit the required KYC form. No problem. Kindly, please be sure the forms are not disabled until time has been allowed to resolve. I'm happy to complete whatever verification is needed, but as you can see, I'm having difficulty completing my steps because it's not extremely user-friendly.

Hi Nicole,
My colleague, Manilene, has already forwarded this to our relevant team. They will check on your account, and we will get back to you as soon as we get an update from them. In the meantime, on the email you received, there should be an orange Start the KYC Process button. Clicking it will redirect you to the verification form you need to fill out.

Upon checking, we haven't received your submission. Can you try to fill out this questionnaire again? If the form says, you can only submit it once, you can upload anything requested here as images or send an email to us at kyc-review@jotform.com with the subject Ticket #31073761 and include any requested information.
We'll be able to help you once you submit the form.

Rico/Jotform, I just went to submit the form again and received this screenshot (below and attached) stating only one entry is allowed and that multiple submissions are disabled for this form. Again, Im fine to allow Jotform time to resolve the issue, !!but please re-enable my forms in the meantime.!!
