I reached out to our IT department about the authentication error. They have confirmed that there is no issue from AD side, the authentication was successful. Please see attached screen shot when I logged in into JotForm.

Hi Deanne,
I'm trying to log in into the Enterprise account, but I get the same authentication error.
When I tried the Starter account, I could get into the account, but it does not have the forms we used to have and looks like I am not an admin.
