How to send forms to a patient?

You can easily email forms to your patients for them to fill out and return. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the screen, tap on Publish.

2. Under the Publish tab, tap on Email.

3. On the screen that opens up, tap on Share on Email.

4. Then, tap on the Recipients tab, and enter the email address you want to use.

5. Once done, tap on Send at the bottom of the screen, and click on Done.

Thanks for reaching out to Jotform Support. You can use the Share on Email feature and add a message manually. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Publish.
2. Under the Publish tab, click on Email.
3. In the screen that opens up, click on Share on Email.
4. You can then manually enter the email content, then click on the Recipients Tab and add the email address in the Recipients Section. Finally, click on Send.
