Unable to add additional fields to forms

Good morning,

We have attempted several times to add on an additional field to our Client Intake Form and the Spanish Intake Form.

Client Intake Form: additional field right below Primary Clinic named - Additional clinics/hospitals visited within the last year.

Spanish Intake Form: additional field right under Clínica Primaria named - Clínicas/Hospitales adicionales visitados en el último año.

Once the field is "added on", it does not save + reflect on the form we have on our tablets.

Please help us with this matter.

Thanks for reaching out to Jotform Support. Under the Bronze plan, you can use up to 250 fields per form. I checked both of your forms, and they are still within the limit, so there should be no issue adding another field. I was also able to locate the fields successfully. Take a look at the screenshots below to see my results:I went ahead and cleared your form cache, as this typically fix minor form issues. Can you go ahead and try it again and let us know how it goes? If you’re still having the same issue, we’ll look into it more to see if we can figure out what’s going on.
