Thanks for using Jotform. When I checked out your account, I noticed that your account was suspended by our anti-phishing system because your form titled Appointment Request Form includes fields asking for sensitive information, which violates our Terms of Use. For us to lift the suspension, we’ll need your confirmation to remove the relevant fields from your form.
In line with our PCI Certification, we have to deny the collection of credit card details on regular forms. But, we have other PCI-compliant ways you can do this. All you need to do is choose one of the Payment Processors available on our platform and integrate it into your form. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of our Payment Authorization feature.
Once we hear back from you, we’ll be able to help you with this.

Hi e50627978,
I removed the sensitive data collection field from your form and reinstated your account. You should be able to log in now. You can stop your account from being suspended again, by making sure that your future forms comply with our Terms of Use.
