Inconsistencies with my form

Please review my conditions in my form Health Wealth & Self - Easy Med Refills (FULL FORM) - Production and help me with the correct order. I need to ensure that the order of my medical profiles conditions and global conditioning is not creating inconsistencies with how my form is displayed. My form has 25 medical profiles. There can be identified by their DnMn naming conventions. Each set of D1M1-D1M5, and D2M1-D2M5 through D5M1-D5M5 are their own medical profile and each should have the same conditioning. I need to make sure that the condition with in each medical profile is in proper order. Please check the order of D1M1. If there a issues with order and/or redundancy let me know so I can build an efficient form.

Thanks for reaching out to Jotform Support. I cloned your form and noticed that it has 470 conditions. Can you share what the form should do with the way these conditions are currently set up? It would also help if you could explain how we should fill out the form to properly test the D1M1 profile so we can check the order and identify any issues or redundancies.
