    Can you resend me the verification form to my email? My jotforms are locked. I never got the email for verification, can you email it to karikleingartner@redrivercomm.com?

    The form has been temporarily disabled due to the collection of sensitive data, such as Personal ID Information. Collecting PII without verification is not allowed. Our top priority is to ensure the security and privacy of all our users. Also, the account owner should receive an email about filling out a questionnaire to verify their organization. If you received an email about your form(s) collecting PII, please note that account verification is required to comply with our Terms of Use. To verify your account and become a KYC-approved user, follow the instructions provided in the email. If this isn’t what you’re referring to, kindly share more details so we can assist you better.

    Hi karikleingartner,

    Thanks for using Jotform. When I checked your account, I noticed that it was auto-suspended by our system because your form titled CLEC Residential - Full Application includes fields to collect sensitive information, which violates our Terms of Use. For us to lift the suspension, we’ll need your confirmation to remove the relevant fields from your form.

    Once we hear back from you, we’ll be able to help you with this.
