can you suggest a form that I can buy for my website that looks similar to the one that is currently on my site but it needs to include a description box where the customer can type their needs and questions and then have the submit button send it to my email address?

You can create a form that closely resembles the one on your website with a description box for customers to type their needs and questions, and a Submit button that sends the information to your email address.
