"Energy Boost Sublingual" Pre-Survey Form Experiencing an Error

    When form has been completed, it is not allowing it to be submitted. When the "Submit" button is clicked, an error message is appearing.

    Thanks for reaching out to Jotform Support. Can you confirm if this issue is happening for all users who submitted the form recently? For now, I've gone ahead and cleared your form cache to fix any temporary loading issues, so could you try a test submission and see if that helps? If the issue persists, could you also try a test submission using a different browser?
