Hi Osko,
I cloned your form and tested it, but I couldn't replicate the issue. Check out my screencast below:
- In Smart PDF Forms, click on Publish on the blue navigation bar at the top of the page.
- From the Quick Share section, click on the Gear icon at the top.
- Select Public from the Access Settings.
Once we hear back from you, we can help you sort this out.

Hi Jonathan,
I have found that the conflicting conditions causing the specific values to appear twice on the Foundation Risk Contributors field are due to the Wood Construction field being included in the condition below:
To fix this, you may need to remove that field in the mentioned conditional logic involving the Wood Construction field to prevent repeated entries, and then add it back. Let me show you how:
1. In Form Builder, click the Conditional Branch icon of the Wood Construction field.
2. Then it would open the conditions connected to that field, click on the Pencil icon to edit the condition.
3. Now, delete the Wood Construction field by clicking X. When it gets removed, click on Save.
4. While on the Conditions page, click on the Pencil icon again to edit it, then add the Wood Construction field again, and Save.
