Hi Dave,
Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, and I ran into the same thing. When I looked at your form, I saw that you created a condition to only show the BandMid field if the Admin PIN entered is FIXQ. Check out the screenshot below:
Let me show you how to fix it:
- In Form Builder, select the BandMid field and click on the Conditional Logic icon on the right side of the field.
- In the Conditions tab, hover over the 2nd condition and click on the Pencil icon beside it.
- Remove the BandMid field in the Field section inside the Show/Hide Field condition and click on Save.
That's it. Test your form, and you’ll see that the BandMid field now appears as expected. Check out the screencast below for the result:
