Form not converting to pdf correctly

    We send out a pre-operative questionnaire to all clients the day before surgery. When they fill out the form, it comes to our email in a pdf form we print out for the surgeon. Ever since we switched to using jotform recently, the pdf comes messed up. I have attached one example, the answers do not line up with the questions. How can this be fixed? I tried to call the AI support agent and it was not helpful, it answered general questions but could not help me with anything specific.

    Thanks for reaching out to Jotform Support. I understand your submission PDFs aren’t lining up correctly, and the answers are overlapping or showing in the wrong places. This usually happens when the fields in your PDF layout aren’t mapped correctly to your form.

    I messed around with it until I saw where I could edit/move around the answer boxes. It is all fixed now, thank you for your help!
