I want to combine my forms

    Hello,

    Our office is hoping to combine the below forms to create a comprehensive questionnaire, which we hope will avoid our patient having to click multiple links.

    Currently, patients are having to complete forms individually, and then we are combining the PDF's of each Jotform as they are emailed to us. Patients sometimes do not complete all 4 links and we are trying to create a system which avoids us having to chase up with each patient regarding the forms they do not complete.

    Our goal is to be able to combine these Jotforms - to allow our patients to complete these forms all within 1 link.

    We would ideally like these forms to be on separate pages, and by the end of the combined questionnaires, we would like to be able to have a PDF copy emailed to us and a copy sent to the patient.

    We have attempted to do this ourselves, however we are having great difficulty.

    Is someone able to please assist us by cloning the forms below and attempting this?

    Hi Roger,

    You can use our iFrame Embed widget, where you can embed all your forms in a main form. Let me show you how to do this:

    1. In Form Builder, click on Add Element on the left side of the page and from the Widgets tab, search for iFrame Embed. I want to combine my forms  Image 1 Screenshot 100
    2. Click on the Gear icon of the iFrame Embed widget, and from the General tab on the right, enter the form URL in the Frame url field.
    3. Lastly, click on Update in the bottom-right corner of the page. I want to combine my forms  Image 2 Screenshot 111

    Here's the one I've created:

    I want to combine my forms  Image 3 Screenshot 122 I checked one of your forms, and it appears that you already enabled the attach PDF option. Note that you'll receive a separate copy of the PDF from each form, since the forms will be sent separately. Another workaround is to combine all the fields so they will all be on one form. You can use our Field Manager app. Let me show you how to do this:

    1. Open the Field Manager app and click on Manage Fields.

    5. On the second form, tick Select All at the top and unselect the fields that you don't want to include.

    On the first form, you can now add a Page Break. Let me show you how to do this:

    1. In Form Builder, hover your mouse over the Submit button and click on Add Element on the left side of the page.
    2. From the Basic tab, scroll down your mouse to the bottom of the page and select Page Break. I want to combine my forms  Image 8 Screenshot 177

    Note that if you have set up a condition on each form, they will not be cloned. Only the form fields can be cloned here. I've already cloned all the form fields from the other forms. Check out my screencast below:
