When I checked your account, I noticed that it was suspended because your forms were marked as risky by our system. The URL where the form is used.
A link to your website or proof of affiliation with a legitimate institution.
Any supporting documentation that can validate your claims such as business registration document or IRS documents.
A valid government-issued ID card, such as a driver's license or passport, along with a clear, forward-facing selfie for verification.

The selfie must be a clear, forward-facing photo of the individual's full face.
