    Hello,

I'm trying to successfully format a document (interview guide) and it looks correct when I edit it. However, when I test it and complete the form it is not coming over correctly. I need some help with formatting the form please.

    Hi Lesley,

Thanks for reaching out to Jotform Support. I cloned your form to see if I could replicate the issue, but everything is working properly. Take a look at the screencast below:

A screenshot of what you’re seeing would be really helpful. Our guide on How to Post Screenshots to Our Help Center shows you how to take one.

Once we hear back from you, we'll be able to help you better with this.

I am not seeing the evaluation criteria and final recommendation areas in your video.

Those are the two areas that are most important to have pulled over from the form to the PDF.

    Hi Lesley,

When I choose original PDF with the Form Submission data PDF file in the notification email, I was able to see the evaluation criteria and final recommendation. Let me share the screencast when I cloned and tested your Interview Guide - MtP- Material Handler form. Let me share the screencast below:

Cant see the number values on the original PDF document using Input Table Image 1 Screenshot 30 I've cleared your form caches; your forms are now refreshed. Can you test again? I noted that you've already selected the original PDF with the Form Submission data PDF file in your notification email. So, you should be able to receive those two fields data as well. Let me share the screenshot below;Cant see the number values on the original PDF document using Input Table Image 2 Screenshot 41Give it a try and let us know how it goes.

This is what I'm seeing on the PDF.

I need to see the numbers in the first screen shot on the PDF.

    Thanks,

    Hi Lesley,

The dot icon that you see on the Original PDF is an expected behavior since you use radio buttons in the Input Table field. If you want to show the number value selected by the user, you can use the dropdown option instead. Let me show you how:

    1. In Smart PDF Forms, scroll down to the Input Table field and click on the Gear Icon.

    2. Go to Fields Tab and select Dropdown from the options.

    3. Remove the other Columns and leave one only.

    4. Add the number options under Dropdown Options.
