お世話になります。
現在アンケート調査のためのフォームを作成していますが、複雑な回答を求める質問の作成が可能か確認したいです。具体的には、例えば同居の家族を聞き、父、母などチェックしてもらい、子どもなど複数の場合はそれぞれ何人かも答えてもらいたいです。また収入を聞く質問では受給している手当ごとにチェックしてもらい、金額もいくらか入力してもらう、などです。
そのようなフォーマットは作成できますでしょうか?

Now, coming back to your question, you can create a form that conditionally show more fields depending on the previous answer, by using our Conditional Logic Show or Hide Fields feature. In addition, you can also add Single Selection or Dropdown field for the respondent to select the allowance they received. In Single Selection field, you can also add Other field, which will allow them to type their preferred answer instead from the selection. You can consider testing our Form Builder directly for free.

Yes, using the features you provided above is possible with the Starter Plan. On the first part, you can use the Fill in the blank element:
Then, in the Fill in the blank field, under Add Fields, you can choose to use elements such as Multiple Choice:
As for the second part, you can use the Input Table Field:
Then use Textbox input type to allow users to enter text into the table:

I still don't understand how to fill in the blank spaces.
How should I proceed to create the attached questions?
I tried creating it as a “multiple-choice” question, but since I can't enter individual numbers, I think there must be a better way.

“Q3 Who are the family members you live with? Please list your relationship to each person and the number of people.”
I want to create a form below this question where respondents can select their relationship and the number of people.

Hi Mayumi,
You can add the field label before the first checkbox. Let me show you how to do this:
- In Form Builder, hover your mouse over the first checkbox of your Fill in the blank field.
- Click on the Number Menu icon at the top and remove the number that will be added.
- Enter your preferred field label.
- Click on Enter from your keyboard so the question will be on the next line.

For number of item, you can consider adding Number filled under Add Field instead.
If you would like to show/hide fields depending on the number input, unfortunately, it isn't possible to Fill in the Blank field.

For checkbox, while there is no specific check field in Fill in the Blank, you can consider adding Multiple Choice instead. It'll give the same action (check) when the user click on it.

Hi.
I already use Multiple Choice.
Since only one father or mother is assumed, a checkbox is sufficient.
For items like grandparents or siblings, there may be none, so I want to allow entering a number, but I don't know how.
In other words, for this Q3, I want both a checkbox and a number input field.
Since the answer below indicated this is possible, please tell me how to do it.

For Q3, you can add both a checkbox and a number input field. Let me show you how:
- In Form Builder, click on add element.
- Under the basic tab, drag-drop the fill in the blank element.
- Inside the drag-drop element, click on add field and select multiple choice to add “grandmother” option.
- After adding grandmother option, click on add field and select number.
- After that, double click on the number field, it will open the left panel. You can use the sublabel field to add a label to your number field
