Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, and I ran into the same thing. When I looked at your form, I saw that the Form Calculation Widget wasn’t configured correctly. Let me show you how to fix it:

1. In Form Builder, click on the Form Calculation Widget, and click on the Wand icon to open its settings.
2. Then, set the formula like you see below and click on Save.

You'll need to click on the operator (+) to make it a number type. This is what it looks like after adjusting the Form Calculation Widget:

Now, coming back to your question, to clarify, do you want to assign a value to each option from the question? If yes, you can enable the Calculation value for the Single choice element. Let me show you how:

1. In Form Builder, select the Single choice element, then click on the gear icon to open properties.
2. Go to the Options tab on the properties on the right.
3. Turn on Use Calculation Values. Under Calculation Values, enter the values of your field options for 1 point.

Agora queria saber como faço para retirar o útimo campo, sem aquele botão "voltar", pois o usuário precisa preencher a página 6 e quando clicar em "Próximo" receber a página de agradecimento equivalente a pontuação.
