I checked the form, and it is under the account do*ap*s*e*ce*@g*ai*.c*m. It is active, and you can log in to it if you're familiar with that email. If you’re having trouble accessing your Jotform account, you can fill out the Jotform Account Recovery Form, and our team will contact you for assistance. You can also reset your password if needed.
