Hi Support Team,
Need your kind investigation to one of our form on JotForm as following:
PTIS - SPTI and LPTI Completion: https://www.jotform.com/72540678300453
Our user submitted a responses from said form and found to be duplicated when checked from submission records. Sample as below:

Regarding the duplicated submissions on your form, this can happen if the form allows multiple submissions without restrictions. Jotform has a feature called Unique Submission that can help prevent duplicate entries by checking cookies and/or IP addresses.
Here’s how to check and set it up:
- Go to your Form Builder and open the form settings.
- Click on the Settings tab and then Show More Options.
- Scroll down to Unique Submission and select either Check Cookies Only or Check Cookies and IP to prevent duplicates.
This will help reduce or eliminate duplicate submissions from the same device or IP address.
Regarding your question about whether duplicates come from the same IP or different devices, you can check submission details in Jotform Tables by enabling the Submission ID and IP address columns to investigate further.

I checked your form settings and confirmed that the Unique Submission feature is already enabled with the Check cookies and IP address option. This setting typically prevents users from submitting the form multiple times from the same browser and IP address. But based on the submission timestamps and remarks you've shared, it appears that some entries are still being recorded in close succession.
This behavior can happen if users refresh the page after submitting the form, especially if the form does not redirect to a Thank You page or confirmation screen. In shared network environments, such as offices or training centers, multiple users may also submit from the same IP address, which can bypass cookie-based restrictions.
To avoid this in the future and reduce the chance of duplicate entries, we recommend enabling the Unique Field feature by setting a required field, such as an email address or ID number, to accept only one unique value per submission. You may also consider adding a CAPTCHA field to help prevent accidental resubmissions and ensure that each entry is intentional. Let me show you how:
- On your My Forms page, hover your mouse over your form and click on Edit Form on the right side of the screen.
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings, and then click on Show More Options.
- In the Form Settings menu, scroll down and select No Check in the Unique Field dropdown menu.
4. Click on the Add Element button on the top left side of the page.
5. In the Basic tab, scroll down and drag and drop the Captcha Element into your form.
