Steps I’ve taken so far:
- Verified the fields are required and filled out on submission.
- Recreated the Notification and Autoresponder emails from scratch.
- Tried using multiple tag variations (e.g., {phone}, {phoneNumber}, {email}, {emailAddress}), but the values are still blank.
- Confirmed that other fields on the form populate correctly.
Could you please investigate and let me know the correct Unique Names/tokens for my Phone and Email fields, or fix why these fields are not passing into the emails? This is critical since I need both pieces of contact information to follow up with applicants

Regarding your issue with Phone Number and Email fields not showing in Notification or Autoresponder emails, please ensure you are using the correct Unique Names (field tags) for these fields. To find the Unique Names:
- Open your form in Form Builder.
- Select the Phone Number or Email field and click the gear icon to open properties.
- Go to the Advanced tab, scroll down to Field Details, and copy the Unique Name.
Use these Unique Names as tags in your email templates (e.g., {uniqueName}). Avoid guessing tag names like {phone} or {email} as they might not match your form's actual field names.
Also, check your Notification and Autoresponder email settings under the Settings > Emails tab to confirm the tags are correctly placed and that the emails are set to include these fields.

Hi Adriel,
For us to investigate the issue, can you share with us the URL of the form so that we can check? You can easily find your form's URL in Form Builder. Let me show you how:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the Quick Share tab, click on Copy Link in the Share With Link section.
Also, take note that for the email address or phone number to appear on the email notification, there must be a field associated with it. For example, there must be an Email field or Phone Number field added to the form for the submission to be added to the email notification. If there are no form fields associated with the unique names, the form won't have a source to get the data from.
