All of my forms have gone missing. I need assistance recovering them.

    Let me help you with that:

    If your forms have gone missing, please check your My Forms page on Jotform. Also, look in the Archive and Trash folders located at the lower left side of the My Forms page. If you accidentally deleted any forms, you can restore them from the Trash folder by selecting the form and clicking Restore.

    Forms in Trash stay for 30 days before permanent deletion. If you don't find your forms there, please let me know.

    I am unable to locate your account using your email address. Can you provide to us the email address linked to your account or you Form URL?

    Once we hear back from you, we'll be able to move forward with a solution.

    Here is one of our form URLs. Low Cost Vaccine Clinic Registration Here is another Adoption Application Dog. And another https://form.jotform.com/animalhouseretail/reptile-adoption Here is another that we have disabled https://form.jotform.com/251834410581150 We have hundreds of forms dating back to 2020 and I can't get the URLs to all of them. There are multiple emails associated with our account. store@animalhouseretail.org, safehaventeamPA@gmail.com, marybeth@animalhouseproject.org and myannessa@refrigerationtechnologiesllc.com are 4 of the main emails. Joseph from Jotform said the email used was diane@ddpets.com. That is a prior old email (we had removed it from our account a while ago) and the person using that NO longer has permission to use our forms. I believe that they have their own separate Jotform account though so I'm really hoping that the two accounts haven't been combined.
