Hi Jotform Team!
I would really appreciate your help. I have did my best to figure it out on my own by following the instructions. I have a 17-question survey that has 3 answers (Yes/No/Unsure).
Based on the added parts, it will show 2 different outcomes.
However, when I finish the questionnaire, it just goes to the Thank You page but does not reveal their results in real time.
Can you please help? I have spent a week trying to figure this out.

Hi toucanhalom,
Thanks for reaching out to Jotform Support. I understand that you’re looking to create a survey form that will show respondents their results right after completing the questions. You can do this by setting up calculation values for each answer choice, using the Form Calculation widget to total the score, and then creating Show/Hide conditions to display the matching result text once the form is submitted.
When I looked at your form, I saw that you've already assigned calculation values to the options in your questions; however, the formula in your calculations wasn’t configured correctly. Let me show you how to fix it:
- In Form Builder, select the Form Calculation widget and click on the Wand icon to open its Properties.
- In the Properties pane on the right, delete all the text in the editor.
- Now, click on the Add Field button to add your questions and set the formula like you see below:
- Finally, click on Save, and you're done.
After that, you'll also need to set up a Show/Hide field condition to show the results based on their score.
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- Click on Conditions on the left of the page.
- Now, click on Add a New Condition and then click on Show/Hide Field.
- Set the conditions like you see below:
- Finally, click on Save at the bottom of the page.
