I'd like to understand why you need an email and a password for the form you've filled out. Do you mean an email or a password to access your personal Jotform account?
I checked your email log, and you requested a password reset. You can check the email address and follow the instructions to reset the password.

Hi Elsie,
I understand you need a refund of your payment. Note that Jotform is only the form provider. The user can create an account on Jotform, create forms, and have users fill them out. They also manage the submission, payments, refund, etc. I suggest that you reach out to the form owner. If you receive a confirmation email after completing the form, you can respond to it and let them know you wish to receive a refund.
