Questions are showing even if hidden by default

i have a conditional logic and its not working. im marking which questions i want to show but its showing everything. i even hid those questions but they are still showing. please help

Thanks for reaching out to Jotform Support. I checked your most recent forms and only found the ABC Intake Form with hidden fields. For example, the fields in the box are hidden by default:

And are only shown conditionally, which seems to be working correctly:

But, if you are referring to something else, can you share the link to the forms and the fields affected?
