Contact information request for a specific contact person to help with questions we may have

I am submitting Jotform as a new vendor at UPITT and the submission requires that I include a Name, Email Address, and Telephone number for a contact person at JOTFORM that Purchasing can reach out to with questions.

Thanks for reaching out to Jotform Support. There are no contact details to get in touch with a specific Support Agent with questions. If there are any questions, they can reach out to us by creating a new ticket from the Help Center, and one of our Support Agents will respond to any questions they may have.
