Hi Ian,
Thanks for reaching out to Jotform Support. I understand you’re having trouble with the Dynamic QR Code on your form not generating a .PNG image. Let’s try a quick fix that usually helps with widget-related display issues.
Sometimes the widget doesn’t refresh properly after edits. You can try removing the Dynamic QR Code widget and adding it back to your form. Before removing it, make sure to take note of the current settings so you can easily reconfigure them afterward.
Give it a try and let us know if you need any help. -

Hi Nazli,
Thank you. Even with just a very simple new form it is not working:

Hi Ian,
I cloned your form and was able to replicate the issue with the Dynamic QR Code widget. When I looked into the widget settings, I noticed that you added First Name, which is not a field label in your form. You can replace it with Name instead to match the actual field label.
If you want to specifically add the first name, consider replacing the Name field with a separate Short Text field labeled First Name. This will allow the widget to correctly generate the QR code based on the proper field.
