Hi Richie,
Thanks for reaching out to Jotform Support. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
I've cleared your form cache to resolve any temporary issue and can see that your form is working fine. Can you go ahead and try it again and let us know how it goes? If the same thing happens again, you can share with us a screen recording of what’s happening so we can try to replicate it on our end. You can share the video recording on any public sharing platform like Google Drive or Dropbox, or you can simply use Loom to record the video and share the video link with us.
Give it a try and let us know how it goes.

In fact, none of the other text is showing

- This forms part of your Due Diligence trail. This information to be kept by Manager / Supervisor for a period of 3 months, within the food safety filing cabinet / files ready for inspection. If the temperature on checking is not within the above parameters, the fridge or freezer and food must be checked one hour later, if the temperature is still incorrect, you must contact the manager / supervisor and inform him / her what items are being stored so the correct action can be taken.
CORRECTIVE ACTION KEY CODE:
A: DEFROST CYCLE
B: NOT IN USE
C: AWAITING REPAIR
D: GOODS RECEIVED WITHIN ONE HOUR

Hi Richie,
By default, Header, Image, and Paragraph fields in the form are not included in email notifications. Only fields where the user provides input are automatically shown in the email alerts. If you'd like the content of a Paragraph field to appear in the email notification, you’ll need to manually add it to the email template. Let me show you how:
- In Form Builder, in the orange navigation bar go to the Settings.
- In the left panel tab, go to Emails.
- Hover your mouse going to email notification, and click the Pencil Icon.
- Modify your email content and place the text. Click on Save and you're done.
We also have a guide on How to Edit Notification and Autoresponder Emails that you can check out.
Give it a try and let us know if you need any help.
