Embedded link showing an error

Hi,

I created this form https://form.jotform.com/251967901534059 and it has been working normally but I just embedded the form into my work SharePoint and now I notice that when I test the elements, I see an error. I dont know if this has anything to do with the error but I didnt notice it before embedding.

When I put the number of travelers in as 5, it should give me the option to enter the names and email addresses of all 5 travelers. I set up the conditions to show the email and name section correlating to how many travelers there will be.

Please see below screenshot of an example of the error. I entered 5 travelers, but the Traveler #4 name is missing. I checked and everything looks normal to me in the set up.

Thanks for reaching out to Jotform Support. I tested your form and was able to replicate the issue. When selecting 5 travelers, the "Traveller #4 Full Name" is not being displayed.

Upon checking your form’s conditions, I noticed that the condition for 5 travelers is incorrectly set to show "Traveller #5 Full Name" twice, while there’s no action to show the "Traveller #4 Full Name" field.

I updated this condition in a cloned version of your form, and everything worked correctly after that. Please refer to the screencast below:

Let me walk you through how to update the condition in your form:

1. In Form Builder, select the Number of Travelers field and click on the Conditions icon.
2. Hover over the condition related to 5 Travelers and click the Pencil icon to edit.
3. Scroll down and update the action to show Traveller #4 Full Name Field.
4. Click Save to apply the changes.
