Hi Vallabhi,
Currently, a slider with text labels for the low and high rating that registers numerical input isn't supported. The feature to do that isn't currently available, but I can see how useful that could be.
I've gone ahead and submitted a feature request for that, and we'll be sure to follow up with you here if that functionality is added in a future update. In the meantime, I can walk you through a workaround.
Using our Scale Rating element, you can describe the Lowest and Highest values. Let's walk through setting this up together, below.
In Form Builder, under the Build tab, click the. +Add Element button.
Click and drag the Scale Rating element to the form.
Change the Field label to "When it comes to transforming how our team operates, our leaders are..."
Click the Gear Icon.
Select the Options tab.
Under Lowest Rating Text, enter "Mostly looking for others to change (value 1)"
Under Highest Rating Text, enter "Eager to be challenged and change themselves (value 10)".
