How can I create a pdf report that sorts my form responses by category?

I've created a form and have got 4 submissions in a table as a result. I would like to be able to have the responses show up in a report that sorts the answers, like this:

1. (first question)
2. answer a
3. answer b
4. answer c
5. answer d
6. (second question)
7. answer a
8. answer b
9. etc.

I don't need to have the outline format (a. b. c.) -- just the answers. Thanks!
