I clicked the link and the form looks fine on my end. Try a few of these methods to see if it fixes it.
- Clear your form cache by going to your My Workspage page, click on your profile, click settings, and then the clear cache under the Form Cache section.
- Try using a different device or network to rule out any local issues.
- Make sure your browsers are fully updated.
- Last thing, try duplicating the form and see if the issue is still there.
If none of those work, reach out again and a Jotform Support member can take a deeper look at what's going on.
