    We need a forms/survey platform that we can use for an average of 50 responses per month. The plan must have the feature to turn off the collection of IP addresses. It would also be helpful to have the ability to generate a QR code for the form.

    Hi Elizabeth,

    The starter (free) Jotform plans can create up to five forms and receive up to 100 monthly submissions. This limit resets every 1st day of the following month at 12am EST. You can learn more about the Jotform account usage limit at this link here.

    Jotform does have the ability to generate QR codes that users can use to access your form(s). Please see the link below for more information on using the QR code widget that is available within the platform. https://www.jotform.com/features/qr-code-forms/

    Lastly, it is not currently possible to disable IP Data of form submissions. However, you do have the option to disable the Submission IP column within your Submissions View section.
