Display issues when form is embedded

When I paste my embed code onto my website it is shown within a box with a different plugin's logo on the top. I have contacted them about it as well. I have tried using source code, Word Press, lightbox as well and each time I get the same result. I use a plug in to design the site, which I disabled and tried my code snippet directly in the text editor, and still it has this box around it.

Any idea how to fix this issue?

I figured it out. I have not idea how it happened but the issue is solved.
