I was able to get it working then when I went to another form and did the exact same setup it wouldn't dynamically pull the information.
I am really hoping y'all can show me what I am doing wrong.

Hi Alex,
Thanks for reaching out to Jotform Support. Please keep in mind that you need to use a unique link generated by the prefill for it to work. Also, ensure that the field values you enter actually do exist in your Salesforce records. If you've added multiple prefill conditions, they should all be true for the form to prefill.
If you think all of this is true, share with us the prefill link and let us know how fields should be filled in, so we could test it and check it further.
