I want to create one form to do the following:
- Collect data
- Collect signatures and initials on a PDF I upload
- Collect payment
Is this possible in one form? I only see that I can do this in two separate forms:
- Via the Form module
- Via the E-sign module

Thanks for reaching out to Jotform Support. You can indeed create one form that collects data, collects signatures and initials on a PDF you upload, and collects payment all in one place.
Let me walk you through how:
Collect Data: Use the Form Builder to add the fields you need to collect information from your users.
- On your My Workspace page, click on Create Form on the top-left side of the page.
- Choose Start From Scratch, and select Classic Form to create an empty form, or Use Template to choose among our 10,000+ ready-to-go templates (Start From Scratch was used in this example).
- Now, in Form Builder, click on the Add Form Element menu on the left side of the page to see the complete list of elements and widgets.
- Under the Basic, Payments, or Widgets tabs, scroll down the list, and then drag and drop the fields onto your form. And that’s it.
Collect Signatures: Add the Signature element or one of the e-signature widgets (such as Signature, Smooth Signature, Adobe Sign, or Docusign) directly to your form. You can also upload a PDF document to create a Smart PDF Form and have users sign it using Jotform Sign, which is fully integrated. Adding the Signature Element to your form is easy. Let me show you how:
- In Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Basics tab, search for the Signature element and click on it to add it to your form.
Collect Payment: Collecting payments via Jotform is very easy, all you have to do is create a form and integrate it with one of the 40+ Payment Gateways that Jotform provides. Let me show you how to create a form and set up a Payment Integration:
- In Form Builder, click on the Add Form Element menu on the top-left side of the page.
- Add the fields that you want to have on your form, and then click on the Payments tab.
- Click on a Payment Gateway, and then click on Connect in the menu on the right side of the screen.
- Once you connect your account in the window that opens, you'll be ready to collect payments through your form.
