Airtable Records Suddenly Stopped

My form (GENERAL MERCHANT AGREEMENT (SIGN) suddenly stopped creating new records on Airtable as of today (as of yesterday). I got no errors or anything. They go through normally on the Jotform submissions. They just stopped generating new records on Airtable and I can't figure out why.

Please help me troubleshoot.

Also, it seems that there are no submissions on your form today. Can you check again and confirm?
