One Missing Field in PDF Submissions

Hello we are not getting all the questions that are asking on our forms to appear on the PDF that we receive. Can you all help with this?

Thanks for reaching out to Jotform Support. I cloned your form to test it, and the same thing happened for me. But after looking through the whole PDF in PDF Editor, I found that the field in question is there, but at the bottom. Check out the screencast below:

You can move the field to the correct position by dragging it. Here's a screencast for your reference:
