Form is showing a different title when being shared.

The form I duplicated is showing a different name when sharing via email/text. I have changed the form Title name under settings and not sure why this issue is still occurring. Can you please advise? Thank you.

Thanks for reaching out to Jotform Support. I have checked your form, and it appears as Cancellation Form, even though the title is Read With Leanne Student Intake Form. But, there's an easy fix for that, let me show you how:

1. In the Form Builder, go to Settings at the top.
2. Select Show More Options.
3. Scroll down to Page Title and enter your preferred title.

That's it, changes are saved automatically. We also have a guide about How to Change the Page Title of a Form that you can check out.
