Hi NSKim,
Thanks for reaching out to Jotform Support. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
I went ahead and cleared your form cache, as this typically fix minor form issues. Can you go ahead and try it again and let us know how it goes? If the same thing happens again, we'll look into it more to see if we can figure out what's going on.
Reach out again if there’s anything else we can help you with.

Hi NSKim,
I also made a test on a cloned form but could not replicate the issue:
Can I ask how are you downloading the submission data so we can try and replicate the issue?
Once we hear back from you, we'll be able to move forward with a solution.

I clicked on the three dots near the tab name and then choose "Download in Excel"

Hi NSKim,
I can look into this for you. I’ll just need access to your account to do that. Don't worry, you can disable that again after we're done checking things out. Let me show you how to do it:
1. In the upper-right corner of your My Forms page, click on your Avatar/Profile Image icon.
2. In the small window that opens, click on Settings.
3. In the panel on the left, click on the Security tab.
4. Then, on the bottom-right side of the page, toggle Jotform Support Access to the On position.
After we hear back from you, we’ll have a better idea of what’s going on and how to help.

Hello NSKim,
Thanks for your patience, and sorry for the inconvenience you have experienced.
I'm pleased to inform you that our engineering team resolved this issue by updating the Download CSV/Excel processes. Now, the column names and arrangements are the same in CSV, Excel, and the Tables Grid.
