And in preview set, theres no title.

Its supposed to be like this :

Can you help me how to change?

Thanks for reaching out to Jotform Support. I understand that your labels of Image Picker items are not showing. When I cloned your form and investigated it, I saw the label is showing but its color is white and the background is white too. If you change the theme or switch the layout from card to classic form; for card forms you can’t inject any custom CSS, probably the issue is sourcing from that.
