Hi Shirized,
Thank you for your assistance. I have two questions about the form: "Hirsch Pediatrics New Family Form" created on March 26.
1)
When we download the form using the first download option the first page of the download is blank except for the following two lines. I would like to remove the extra spaces and remove the words Patient Information from the top.Patient Information

The completed form is e-mailed to us. However in the e-mail the paragraph form elements do not show up. Is there a way to include the paragraph form elements in the e-mail? Currently to include the paragraphs in the form we have to download the completed form and then print out a copy.Thank you!
