My client cannot access the form I made due to my account being suspended. Please fix this immediately.

Hi Maggie,
When I checked your account, I saw that it was auto-suspended by our system because your form titled Let's Begin! MCC Onboarding Process includes fields to collect sensitive information, which violates our Terms of Use. For us to lift the suspension, we’ll need your confirmation to remove the relevant fields from your form.
Once we hear back from you, we’ll be able to help you with this.

Hi Maggie,
I removed the relevant fields from your form and reinstated your account. You should be able to log in now. You can stop your account from being suspended again, by making sure that your future forms comply with our Terms of Use.
