I am hoping to be able to see the sub-labels with name. For example, I would like the pdf to be similar to below:

    First Name: Joe

    Middle Name: Jay

    Last Name: Black Foot

I know I can do this with multiline fields but then it messes up the naming of the pdf's when they get sent to the google drive.

    Hi Mariana,

Thanks for reaching out to Jotform Support. You can edit the PDF document from the PDF Editor to display each field with its sub-label. You can add text elements manually next to each field to replicate the desired format. Let me walk you through it:

    1. Open your form in Form Builder, click on the Downward Arrow icon next to Jotform Logo, and then click on PDF Editor.

    2. In PDF Editor, click on Add Element, located on the left side of the screen.

    3. Under Basic tab, drag and drop a Text element on the document, and then add the fields next to each other.

If this is not what you're referring to, can you share your form URL with us, so we can look into this further.

Give it a try and let us know if you need any help.
