Hi Mary,
It seems you are looking for the old form in your new account. Can you log into your other Jotform account if you have another account? Currently, there is only one form is available in your current account, and it is not the old form. This form is recently got created. In case you don't remember the login details then, can you share with us the form URL in question so, we can try to locate it?
