Upload Files On HIPAA Compliance Form

    Hello,


    Do you have the capability to upload a front and back of an insurance card from your phone or file upload that is HIPPA compliant?

    Hi Ian,

    Thanks for reaching out to Jotform Support. Yes, Jotform allows users to upload both sides of an insurance card via phone or file upload. Uploaded files in HIPAA compliance forms, like insurance cards, are securely stored and encrypted to protect PHI. Simply use the File Upload field in your form and label it, e.g., Upload Front and Upload Back. Let me show you how with my demo form:

    1. In Form Builder, click on the Add Element menu on the left side of the page.

    2. Under the Basic tab, scroll down to File Upload and label it according to the required files.

    3. You can also click on the Gear icon and click on Multiple tab to limit the number of files that can be uploaded.
