Locate the account where the form is

I have created the above form but I am unable to remember or find out which account it was created in.

Thanks for reaching out to Jotform Support. I am afraid, we can't pull up an account using the Document ID. Do you have the Form or Signed Document URL instead?

Hi Jeric, sorry got a little busy.

https://form.jotform.com/250198289301863, I think the account is listed under the email , but when I try and ask for a new password, I dont get a response email. If I log in on the other email which is , I can get in but the templates arent there.

The form is under this email address, s********c*******@g****.c**. If you recognize this email and know the password, you can log in to it. If not, you can do a password reset.
