Issues with the PDF Form

when converting to a PDF some of the sections are in the incorrect area. And I end up with a whole paragraph of my release for above the title for that section. The online form never look that way, but I am trying to attach this form to my client software for appointment and not have to contact each clients individually.

how can I stop this or put things back in the proper order?

I went ahead and cleared your form cache to refresh your forms, can you try downloading your fillable PDF again?
