How to create a form and allow the user to print or download their submission

    Is there a way to


    1. Create a form that user's can download and input information on ( or fill out online and have the information saved from the online input)
      then have that information added to a spreadsheet to track payments and track t-shirt sizes, donation amounts etc.
    2. Allow user's to submit payment for the items put into the form that is downloaded or submitted online

    Basically, the form needs to allow people to enter the names and addresses of attendees to the family reunion, as well as, input the information for T-shirt sizes for each member attending, donations to the scholarship fund etc. give them a total (if the form is online, it would total everything for them and show them the subtotal and then take them to the payment page) then allow the user to upload that information using the form and have that information added to a spreadsheet so the committee can track who submitted payment and also what Tshirt sizes and number of Tshirts will be needed.

    Thanks for reaching out to Jotform Support. Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. Dragging and dropping features or elements make it really easy to create forms.

    1. In Form Builder, go to your My Forms page.
    2. On the left side of the page, click on Create Form.
    3. Click on the Add Form Element menu on the left side of the page.
    4. Under the Basic tab, scroll down the list, choose the options that you want, and then drag and drop them onto your form.

    Additionally, Jotform has any type of Form Element you may need. Take a look at the complete list below and get a brief description of each Form Element in Jotform. Additionally, you can add Print button to your form to allow them to print the completed form. Or, you can Attach their submissions as PDF files to your Notification Emails/Autoresponder email. Let me walk you through it:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on the Settings tab.

    2. In the menu on the left side of the page, click on Emails.

    3. Hover your mouse over the Notification Email element and click on the Pencil icon.

    4. Click on the Advanced tab, toggle Attach PDF to the On position, and select the PDF file you created.

    5. Now, scroll down, click on Save, and you're done.
