Hi,
I am about to launch this digital order form tomorrow https://form.jotform.com/a3bs/3b-scientific-digital-order-form
On page 12, there are lists of additional items they can add (as a multi-select) and if they select some of them, they need to specify a location or another piece of information (using short text). I have included a screenshot of 1 section. How can I add the short text requirement next to the multi-select option they pick to have them specify the information we need (via short text) for that item they selected?
I also have Jotform Support Access enabled for the form so you can login to it if you need to.

Hi Nicole,
To achieve what you want to do you need to Add Short Text Fields for each option in your multi-select list and Use Conditional Logic. Let me walk you through:
1. In Form Builder, click on the Add Element menu on the left side of the page.
2. Under the Basic tab, scroll down the list and choose the Short Text field.
4. Edit the field label based on the selection.
5. Click on Advanced and toggle on the Hide field to turn on.
Now, let me walk you through how to create a condition:
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Click on Conditions in the menu on the left side of the page.
3. Under Condition, click on Add New condition and select Show/hide field.
You need to create a condition for each selection to show the hidden short text.
